Do you know someone who has gone above and beyond to help the well-being of children, youth, and families at Lunada Bay Elementary school or in the local community?  PTA Honorary Service Awards (HSAs) are designed as a unique way for PTA units to publicly acknowledge both individuals and organizations for outstanding community service to children and youth. Current members, officers, community volunteers, Lunada Bay staff and teachers may be considered for this award.  The nominees do not need to hold an official PTA position, nor do they need to be on the Lunada Bay staff.

Please download this form if you would like to nominate someone to receive a Lunada Bay Elementary PTA Honorary Service Award this school year. Please drop the completed form off in the office by March 25^th or email it to Melissa Forrester at melissajforrester@gmail.com. All nominations will be considered. The HSA Nominating Committee will select the recipients.
